Wellbeing Teams are a radical new model for delivering care and support within communities. They are:

  • values-led at every step
  • focussed on co-production, supporting people to make decisions about their life and support
  • committed to wellbeing, both of the people they support and team members

 

What does the well-being team do? We are driven by our values:

 

Compassion: Actively hearing and sensing one another’s thoughts and feelings, being kind, and finding empathetic ways to support individuals and each other to achieve positive outcomes.

 

Responsibility: Initiating ways of working that dignify everyone, at the same time holding self and others accountable for actions and attitudes about our shared purpose and values.

 

Collaboration: Cooperating with others by sharing ideas and insights to find ways of achieving positive change, both individually and collectively.

 

Curiosity: Feeling energised by discovering new insights, learning, finding answers to questions and wondering at the world.

Wellbeing Teams integrate three key elements of care and support to create true wellbeing:

All of these differences – and more – are delivered by set ways of working, which we have developed and are available in our Wellbeing Teams Handbook and other documents. We have also created several ways to learn about what well-being teams do for you.

 

  1. Develop great relationships with people using our services and colleagues, treating everyone with respect, kindness and generosity.

 

  1. Provide practical support to help people live well at home, in the way that the person wants. This will include help with personal care (for example getting in and out of bed, washing, bathing, dressing), providing meals, support with taking medication, looking after the home, and getting out and about.

 

  1. Be part of a self-organised team. This means working together, taking different roles, and creatively solving problems together. You will provide support and cover for each other, give each other feedback and celebrate successes as well. You will have a Buddy, and you are supported by a Team Coach too

 

  1. Manage your time well, and use it in the best way to support people to achieve their outcomes. You have the autonomy and authority to organise your time to do this with your team, the team sets their rotas, and the Team Coach will provide any support that you need.

 

  1. Bring your whole self to work – your talents, interests, and passions. We will support you to pay attention to your well-being, as well as the well-being of your team and the people you support.

 

  1. Collaborate with the person’s family, friends, and Community Circles, as well as any colleagues from health and social care, to make sure that our work is coordinated and effective.

 

  1. Keep all information up to date – the people you support, and your teams.

 

  1. Provide safe, person-centred, compassionate care and work within the team agreements and how Wellbeing Teams work, these are all described in your handbook.

 

  1. Be respectful of people’s homes and possessions, and any equipment.

 

  1. Keep learning and developing individually and as a team so we can keep improving how we work and flourish together.

How Wellbeing Teams work

 

What does the well-being team do? Like traditional care services, Wellbeing Teams provide help with things such as washing, dressing, or medication; but, crucially, they also support people to do more of what matters to them and to stay connected in their communities.

 

Wellbeing Teams use the ‘Support Sequence’ to help people to think about different ways to be supported or make changes in their life. This includes self-care, technology, aids and adaptations, support from family, and friends, being part of what is happening in the community, as well as services and paid support.
